A senior doctor has criticized NHS leaders for persistent issues in England's transplant services, demanding significant changes in a letter leaked to the BBC. Professor Derek Manas, medical director of NHS Blood and Transplant, pointed to a system plagued by workforce burnout, last-minute surgery cancellations, and a lack of accountability, claiming these problems have persisted due to poor management by NHS England.

This criticism comes at a time when the number of heart and lung transplants in the UK has stagnated for three decades. The NHS now performs only one in ten lung transplants and one in seven heart transplants from donations, far below levels seen in other Western nations. The disparity has raised concerns, as the UK was previously a global leader in these procedures. Waiting times for patients can vary dramatically, ranging from eight months to over four years, depending on the hospital, highlighting systemic inefficiencies.

Manas's letter outlines a fragmented system where hospital waiting lists, organ availability, and surgical capacity do not align effectively. He emphasized that NHS England has not actively managed these waiting lists and has failed to ensure fairness in the process. The situation is exacerbated by a significant loss of experienced surgeons, with half of the UKโ€™s heart and lung transplant centres losing their top surgeons in recent years as they seek opportunities abroad. This exodus is contributing to the increased strain on remaining staff and a reliance on goodwill from healthcare professionals.

The issues surrounding transplantation services were first identified in a 2024 report by international experts commissioned by the Conservative government. Although NHS England has committed to improving patient outcomes, many surgeons and health advocates feel that progress has been sluggish. Without urgent reforms, the challenges facing heart and lung transplantation in the UK may continue to worsen, leaving patients with fewer options and longer waits for crucial surgeries.
